You may wish to print this form, and complete it prior to arriving at our practice. Vision Service Plan and other insurance carriers ask that you call our office ahead of time to verify coverage, so that we may explain your options, and answer any questions you may have.
Please arrive at our practice wearing your current glasses or, if you wear contact lenses, please wear them for several hours prior to the appointment, and bring any information you may have on the current prescription (written Rx or contact lens packaging or boxes are sufficient). Thanks.
